The Foundation recognizes that sustainable improvements are driven by locally informed, collaborative solutions. We are committed to partnering with organizations that demonstrate a deep understanding of our community needs, employ evidence-informed and innovative approaches, and engage residents and stakeholders in meaningful ways. We know there is no one-size-fits-all answer, only collective effort grounded in evidence and a shared commitment to improving lives.

Assessment and Planning
We approach all our work with a thorough understanding of the context we are operating within.
Like a tree whose roots reach deep to understand the soil before growing upward, our model for change in community health begins with thorough assessment and careful planning. We ground our actions in the real conditions and needs of the communities we serve. Our solutions tackle big picture issues via whole-systems thinking.
Implementation and Monitoring
Without the trunk, we’d have no branches. Through strategic investments, we implement programs, support policies, and build up our community. At the same time, we listen to what’s working and what’s not. Monitoring allows us to detect challenges — a little more water here, more sun there. The key to our iterative process is continuous feedback, allowing our tree to stay healthy.
Multi-level Intervention
Our interventions are where our work branches out. They operate at different layers — individual, family, community, policy — just as branches spread outward to cover different areas. Through program development and strategic investment, our work branches into the future. These activities yield new growth and innovation that builds toward the thriving canopy of our shared future.

Strong partnerships keep the work grounded, Our collaborations are deep-rooted. We understand that the longevity and sustainability of our work depends on intentional stakeholder engagement. Like roots, our growth is a repeated process of responding to our environment. Through ongoing learning, we strengthen our foundation and reach.
